In your MSMP do you have configuration for Step 1 for Escape conditions (one for when approver not found and the other for when access could not be provisioned)? I assume you do not have the second escape condition configured.
As to why it could not be provisioned you will need to have a look at what sort of access was being requested for that step. Possibly the object was locked or role didn't exist, etc.
